Informatie voor verwijzers

In our program, we see people with persistent back and/or neck complaints for which there are no medical treatment options and the spine is fully loadable. People with a neurostimulator (Spinal Cord Stimulator) are also eligible for participation, provided it is properly adjusted.

Inclusion Criteria

• Persistent spine-related pain complaints
• The spine is medically fully loadable
• Adults (from 18 years) who, due to persistent complaints, are stuck in daily functioning
• See possibility for behavioral change, for the benefit of improving functioning
• Good understanding and command of the Dutch language
• Ability to function independently

Exclusion Criteria

• Medical diagnostics not completed
• Current addiction issues (including drugs, alcohol)
• Prominent psychiatric issues
• ABI (Acquired Brain Injury) or other (neurological) conditions that make participation in intensive treatment inappropriate

General practitioners can refer via Zorgdomein:

When referred via Zorgdomein, a special route will be started via the orthopedics department of the Sint Maartenskliniek towards RealHealth.

Company doctors and medical specialists working outside the Sint Maartenskliniek may send or email the referral, including the complete patient data, directly to RealHealth: realhealth@maartenskliniek.nl. If available, please include existing imaging. The secretariat arranges a medical consultation at the Sint Maartenskliniek, after which the further intake procedure at RealHealth can be started.
